The {{c1::spinnaker sail sign}} is seen in {{c2::pneumomediastinum}} and represents the {{c3::thymus lifted off the mediastinum}} by the ectopic air. 
Extra Info
Do not confuse this with the sail sign, which is the rightward extension of the normal thymus.
