Longitudinal Erythronychia and Squamous Cell Carcinoma (SCC)- what is it? DDx? Most common nail unit malignancy? Associated with which HPV?

Focal loss of nail plate production causes thinning of ventral nail plate, nail
bed vasculature swelling, pinching with possible splinter hemorrhage, distal
chipping, splitting, and hyperkeratosis
• Differential diagnosis: onychopapilloma (67%), glomus tumor (6%), wart
(5%), squamous cell carcinoma/SCC in situ (3%), rarely melanoma
• SCC most common nail unit malignancy, male predominance, 7th decade of
life, fingers > toes
• SCC associated with human papilloma virus (HPV) 16
◦ 24% of patients have other evidence of high-risk HPV infection
• MMS standard of care for SCC (recurrence rates 0-8.5%; occasional studies
with 20% recurrence), amputation should be reserved for bone invasion not
amenable to local resection
