A 65 year old man is 3 days post laparoscopic cholecystectomy. You are called to review him for acute urinary retention. The nursing staff have tried to insert an IDC 3 times but have failed and he is distressed and in pain.

What is your management of this patient, and what is your approach to the difficult catheterisation?

You put an IDC in, however, the nurses now tell you that the patient’s bloods have just come back and the potassium is 6.9 mEq/L, and eGFR is mL/min/1.73m2. He has had IVF running post operatively with 20mmKCL. The patient has a past medical history of type 2 diabetes and chronic kidney disease. You ask for an EKG which showed peak T waves.

What is the aetiology of the hyperkalaemia?

How could you manage this patient?
