A 65 year old man comes into the emergency department with severe perianal pain. The emergency doctor tells you he thinks the patient has a perianal abscess. When you review the patient you note that he has erythema and induration in the perianal region which extends anteriorly into the scrotum and into the left inguinal region. The patient complains of severe pain, is febrile with a temperature of 39.1 degrees Celsius, HR  of 120, blood pressure of 85/40mmHg, respiratory rate of 22 and saturation 98% on room air. He is morbidly obese with poorly controlled type 2 diabetes with a current blood glucose of 19 mmol/L.
