Advance Care Planning Options
*patient wishes + medical orders
Patient Wishes (created by {{c9::patient}})
{{c4::Living Will}} allow patients to identify the kind of health care they would or would not want if (1) they had a {{c2::terminal}} or {{c2::irreversible}} illness, and (2) they were not able to make {{c3::decisions}} for themselves at the time.
*typically {{c10::end of life}} decisions
{{c4::MPOA}} gives authority to a surrogate healthcare agent to make medical decisions on their behalf if they lose {{c1::capacity}}
*does not need to be a {{c2::terminal}} or {{c2::irreversible}} illness, can just be any health care decision
Medical Orders (created by {{c9::physician}})
{{c6::DNR / DNAR}} is a medical order consented to by a patient or surrogate and entered by the physician that directs the care team not to perform the standard procedures for {{c5::resuscitation}}
{{c8::POLST / MOLT}} is Physician/Medical Orders for Life Sustaining Treatment → useful for patients with serious, terminal, or irreversible illnesses to discuss their {{c7::wishes}} with physician
