List the factors which increase the risk of stomal recurrence in patients receiving post-operative radiotherapy after total laryngectomy for laryngeal cancer. (3 marks)

Patient: 
- Age 
- Smoking 
- Use of radiation sensitising medications eg methotrexate
- Co-morbidities that affect healing eg type 2 diabetes
- Radiation sensitising conditions eg ataxia talengiectasia, connective tissue disease, nejmegen breakage syndrome

Tumour: 
Tumour location: subglottic (most common) > glottis > supraglottic 
- Tumour differentiation: poorly differentiated > well/moderately differentiated  
- T stage: higher (particularly T3/T4) > lower 
- N stage: positive > negative 
- Pharyngoperistomal fistula: present > absent 

Treatment: 
- Preoperative tracheostomy 
-  Surgical margin: positive > negative 
-  Interrupted/increased OTT for RT 
- RT geographical miss or underdosing
