Pathoma
In scarlet fever, the rash appears within 24–48 hours of the onset of upper respiratory infections. It begins around the head and neck and spreads over the trunk and extremities. It usually spares palms, and soles. It is a diffuse, papular, and erythematous eruption, which blanches on pressure. The cheeks are erythematous, with pallor around the mouth. The tongue is usually coated, and the papillae are swollen, giving the tongue a strawberry appearance.
The image below shows a strawberry tongue.
