In PAH patients who have a positive vasoreactivity test, {{c1::calcium channel blockers}} are indicated for first-line treatment.

Vasoreactivity testing involves the administration of inhaled nitric oxide, IV epoprostenol, IV adenosine, or inhaled iloprost.
The test is considered positive if the mean pulmonary arterial pressure (mPAP) decreases by at least 10 mmHg and to a final value ≤ 40 mmHg, without a decrease in cardiac output.
