Is gout more commonly the result of the underexcretion or overproduction of uric acid?
Gout is caused by underexcretion of uric acid in 90% of cases

Renal underexcretion of urate in 90% of cases
 Overproduction of urate in 10% of cases 
There is often an overlap of both

Causes of underexcretion:  idiopathic, CKD, obesity, HTN, low dose ASA, alcohol, diuretics 

Causes of overproduction:  idiopathic, inherited enzyme defects, certain heme/lymph dz, purine-rich diet.
