Myocardial Oxygen Balance and anesthetics' effect

• Most oxygen consumption is due to pressure work
• Myocardium extracts 65% of O2 in blood (compare with other tissues that extract 25%)
o Therefore myocardium cannot increase O2 extraction in times of inadequate blood flow
o Thus, heart rate determines both O2 supply and demand in the heart
• Anesthetic agents
o Coronary vasodilation and reduction of metabolic requirements
o Reduction of arterial blood pressure, decrease preload and afterload
o Protection against reperfusion injury after ischemia
