• When myocardial O2 demand > supply
• Atherosclerosis: cholesterol deposits beneath
vascular endothelium → calcification →plaques that obstruct blood flow
o Plaques can lead to a clot (thrombus) that occludes the artery
o Plaques can rupture (break away) and flow until they block the artery (coronary embolus)
o Plaques can irritate vascular wall →
vasospasm (wall contraction)
