What is the main benefit of drug eluting stents vs bare metal stents?

Drug eluting stents (DES) contain an antiproliferative drug to inhibit growth of the neointima, preventing restenosis.
- Common medications used include: siroluiimus, paclitaxel, everolimus, zotarolimus
- Superior to bare metal stents regarding need for repeat vascualrisation
- No difference in long term mortality or late in-stent thrombosis rates compared to bare metal stents

DES reduces rate of early in-stent thrombosis and reduces rate of repeat revascularisation. They do not reduce mortality rates or MI rates. DES increase the rate of late and very late restenosis, due to delayed endothelialisation (>1 year) and may require prolonged DAPT
