What are the indications to transition to insulin? What is the regimen early on and later?

Consider in newly diagnosed patients with any of the following:
Initial glucose ≥ 300 mg/dL or HbA1c > 10%
Symptoms of hyperglycemia
Signs of a continued catabolic state, e.g., weight loss
When patients undergo hyperglycemic crisis
Begin with basic daily insulin injection (long acting insulin) - move to twice daily injection
From there move to prandial insulin (short acting insulin) - post meals.
