SS_TS 1.21 Outline the management of the following postoperative complications associated with thoracic surgery:
- Pneumothorax  
Identify if tension pneumothorax -> suspect if hypotension, tachycardia and respiratory distress. Associated with tracheal deviation away from affected side as a late sign 
Treatment
  • Decompression by lateral thoracostomy in 4th/5th intercostal space anterior to mid axillary line. Needle decompression mid clavicular line 2nd intercostal can be used
Pneumothorax depending on side may need
- Conservative management : observe, repeat CXR 
- Chest drain 
- If persistant ? bronchopleural fistula : may need surgical management
